Opt-out affidavits for Medicare signed by physicians on or after June 16, 2015 will be automatically renewed on June 16, 2017 or two years after the effective date. To cancel the automatic renewal, physicians must submit a letter in writing at least 30 days before the start of the next two-year opt-out period to Noridian, Medicare’s administrative contractor for California. Opt-out affidavits signed before June 16, 2015 will expire two years after the effective date with no automatic renewal.
