The Accreditation Council for Graduate Medical Education (ACGME) has created the Clinical Learning Environment Review (CLER) Program to address key areas of patient safety training for residents and fellows. The program resulted from a study that found many physicians-in-training were unaware of the range of conditions that define patient safety and did not understand how clinical learning environments use reportable events to improve systems of care.
